Evaluation Design Framework

Design an evaluation framework for the following government program:

Program: Community health worker (CHW) home visiting program
Target population: Residents with chronic conditions in
underserved neighborhoods
Program activities: Regular home visits, health education,
care navigation, social determinant screening and referrals
Budget: $1.2M annually
Scope: 500 participants per year

Evaluation framework:
1. PROCESS EVALUATION
   - Is the program being implemented as designed?
   - Key process measures (visits per client, referral completion
     rates, caseload management)
   - Fidelity assessment methodology
   - Staff performance indicators

2. OUTCOME EVALUATION
   - Short-term outcomes (knowledge, behavior change, service utilization)
   - Intermediate outcomes (health metrics, reduced ER visits)
   - Long-term outcomes (health status, cost savings)
   - For each outcome: measure, data source, collection frequency,
     target, and baseline
